Application Forms – Fillable PDFs
Completed application forms and ancillary documents should be sent to the relevant Trustee or QFM in the case of 3rd party pension accounts, or directly to Conexim at email@example.com for all other account types. Application forms can be signed using your preferred digital signature system (eg DocuSign) or in the traditional way by wet signature which we will also accept by way of scan.
If you are looking to open an account type that is not listed above, or are having difficulty with the above forms, please feel free to contact your primary relationship in Conexim, or e-mail firstname.lastname@example.org.
Each Application form has a detailed check list on the front page which should be used in conjunction with our AML Documentation Requirements. You should complete the relevant Application form with your client(s) and ensure they receive the additional ancillary documentation below:
